Change Address/Name Application Form

All requests for changes to name and address data used in all Council managed systems should be submitted in writing using the below template form.

Forms can be:

Delivered to: 36 Linsley Street, Cobar
Posted to: PO Box 223, Cobar
Faxed to: (02) 6836 3964

Please note that all forms should be signed by all property owners, animal owner or debtor.
